Who needs form UD-150?
Form UD-150 requests a trial for the party who wants to complain about an unlawful detained. A landlord usually requests this kind of trial when they are not satisfied with tenants and have legal reasons for eviction. Tenants in turn use this form when they want to file a counter-request.
What is form UD-150 for?
If a plaintiff filled out this form according to all the requirements, the court is obliged to respond to the request and arrange a trial for unlawful detained. After all the fees have been paid, the court will appoint a judge and set a date for the trial. This process could take a while if the case will involve a jury.
Is it accompanied by other forms?
There is one addendum to form UD-150 -- it is included as the second page of this template. A party who requests a trial or counter-trial must fill out a proof of service by mail (second page) and send it alongside form UD-150.
When is form UD-150 due?
The due date to file a copy of UD-150 and request a trial must be appointed by the court that approved the detained. When the court clerk accepts a properly filled out copy of UD-150, it takes 20 days to set the date for the trial.
How do I fill out form UD-150?
On the first page, the plaintiff or his legal representative must furnish general information about his case and the trial he requests (its duration, jury or non-jury, estimated length). The second page is dedicated to proof, where the plaintiff must provide the list of names and addresses for each party of a future trial.
Where do I send it?
The requesting party must send one copy of UD-150 to each party of the future trial and to the court where the case will be heard.
